I'm pretty sure the logic for the AI is something like:

1. Figure out what the player wants you to do.
2. Make him go through some convoluted configuration steps to get you to do that thing.
3. Do it perfectly until the precise moment he stops paying attention.
4. Suddenly catch on fire, roll in forgotton beast blood with horrible syndromes, then run into the booze stockpile and get a drink. You're pretty thirsty after all.
5. If you survive, return to work as though nothing happened.

I am trying to make chickens lay bees as eggs. So far it only produces a single "Tame Small Creature" when a hen lays bees.
Honestly at the time, I didn't see what could go wrong with crowding 80 military Dwarves into a small room with a necromancer for the purpose of making bacon.

Quarry bush leaves in general, or quarry bush leaves in bags?

As long as it's not in a barrel, it doesn't matter.

Though I prefer to free up my bags anyway, I like looking at quarry bush leaves in my stockpile.

Quarry bush leaves in general, or quarry bush leaves in bags?
I believe it was leaves in general, whether or not they were in bags, and whether or not the bags were in barrels.

I didn't test for in/out of bags specifically, but I'm pretty sure they wound up out of their bags enough in my testing for that case to have come up.

I can't see why that wouldn't work. You'll probably end up with a lot of job cancellations when your cook gets good enough to cook the meals faster than the ingredients get hauled. I guess what I would do is set up multiple kitchens this way so when one job is cancelled he moves to the next workshop.

A bit of necro, but I didn't want to start a whole new thread on this.  I can confirm that this works, even with booze, dwarven syrup, and quarry leaves.  Thanks for the idea!  I now have four kitchens churning out meals with 4 separate ingredients, and with workflow, it's all automated. 

One thing to note is that quarry leaves might not work depending on the level of your farmers.  When my fort was younger, quarry bushes were harvested in smaller stacks, so each bag of leaves would have multiple bunches.  Each separate stack in a bag would get used, breaking the system.  Now with more experienced farmers, quarry leaves are processed in stacks of 20-25 leaves, with a single stack in each bag :).

Finally got a similar setup to work! I think there's a couple of things worth mentioning for anyone else trying to figure it out.

1) No solid food stockpile can have barrels (even the "main" one(s) that the 1x1's are taking from)
2) Have a taking stockpile to clear away the empty bags (from quarry bush leaves) and barrels (from booze/syrup) to speed things up
3) A 1x2 stockpile accepting 2 barrels works for booze if you want to add wine to your cooking. (I imagine it's the same for syrup/milk etc)
4) Cooks will always (it seems) take items in the same order, so you can make e.g. your meat ingredient last and therefore name every meal after it

I've got a 4-stockpile setup working well:
1) 1x1 Meat/prepared fish
2) 1x1 Tallow/egg/cheese
3) 1x1 Quarry bush leaves / other plants
4) 1x2 Wine (of various flavours)

The haulers don't quite keep up with the cooks, but workflow requeues the jobs without too much spam. When I run out of tallow & eggs I switch to stews.
