We are all familiar with the Metamorphica, I hope? If not,
check it out at once!
In a secret facility maintained by your usual secretive government agency/mysterious conspiracy/mad scientists, years of experimenting have produced the ultimate super serum. A new era is beginning, humanity ready to ascend to heights of science never seen before! The serum allows nearly unlimited change of the human form and mental capabilities - allowing the creation of super-soldiers, super-scientists, super-humans in general. There's only one problem.
Nobody can actually control the serum.
A number of unfortunate scum rounded up on the streets (or worse, volunteers), have been dragged screaming into the depths of the facility and injected with this very first version of the super serum. As everything goes to hell at once, they find that they are free to leave whatever tank of suspicious chemicals or operating table they were strapped into, and that they have undergone some... interesting changes. Unfortunately, these changes show no sign of stopping, and the facility is in full alert...
I use SeanMirrsen's Progressive RTD system as the base. You need not worry your pretty little heads with the details. There are 7 stats; Strength, Agility, Toughness, Intelligence, Perception, Charisma and Willpower, which all start safely at 6.
Everyone is assigned three random mutations from the start, rolling in the Metamorphica. Every 3 turns, I roll a 1d6 for each player. If the number is less than their current amount of mutations, the player loses one mutation. If the roll is the same as their number of mutations, one mutation changes into another. If the roll is more than their number of mutations, they gain a new mutation.
You will be trying to escape the facility, evading the guards and security system, while desperately hoping your next mutation is something like superstrength instead of, say, 400. Needless to say all mutations will be represented in some form, and it's up to you to find uses for them even when they seem totally and terrifyingly useless and/or harmful.
Oh, I just need a name and a gender if you want. I won't mind if you cook up a hugely elaborate back story and motivations, but it's not strictly speaking necessary.

I expect everyone to die horrendously in the first ten turns, so don't be afraid to sign up for the waitlist. The more the merrier! I'm not going to coddle my players like some other weakling GMs! Bring on the lit kegs of explosives, steaming pits of acid filled with mutant, acid-resistant, flying piranhas equipped with flamethrowers and axes, venomous, mechanical, missile-launching morris dancers armed with liquid nitrogen harpoon guns riding giant, rabid killer bees, backed by enormous death rays - and
I won't forget to turn the lightning generator on.*
*Ah, what a wonderful reference.
