Mushroom Kingdom Wars. I didn't use the system much mostly because I was too lazy to set the DC to anything but 10. Basically, if you roll 10 higher than the DC on a roll or more, it's automatically a critical hit, and if you roll 10 lower, it's a critical failure. So if you're trying to break down a steel door (DC 20 or something) and roll a ten, you most likely break your hand and take damage Maybe that's not a terrible system, but there's a reason it's gone through several revisions since then.
