Other. Get a metal thermos, insert milk. Lower thermos into bath of liquid lead. Leave or some time, then remove and pour into glass pre-prepared with chocolate powder. Enjoy.

Insert thermite.

Get 1tb cocoa powder and 1tb sugar (or less, mostly I use 1tsp but I'm not fond of sugar stuff) and mix with tiny bit of boiled water until you can mix it into a paste. Add hot water until the cup is 2/3 full then add milk to top. Not the 'best' method but doesn't rely on you having to get pots out and being complicated.

I think the problem here is keeping a bar of chocolate around long enough without eating it.

I think the real problem here is not inserting enough thermite.

While i totally support the thermite options, i recognize that in some cases it isn't viable(e.g. when you have an ikea desk, and your hot mug tend to pass throught the desk and spill all the delicious study-supporting chocolate on the not-so-in-need-of-warmth floor.).
So i'll go with a less fiery receipe:
1)Put cocoa powder in a little pot, add sugar if you feel sissy like that.
2)Add to the powder(less than one teaspoon for each tablespoon of cocoa circa) some flour, FLoUr?! you'll say. Yeah flour, it add to the "creamyness" of the concoction and give it "presence", just don't exceed.
3)preheat the milk in microwave
4)put the pot on stove, low heat
5) add the warm milk little by little, always mixing
6)raise heat, take to nearly boiling, then mix until the consistency reach your liking. keep in mind that when you stop heating it tickens a bit
7)whip some cream, put it on top, done.

and add MAGMA if you feel like a man(seriously, never tried a little hot pepper sprinkle?)
