After hell hmm good question. Could go the multiple planes or domains of hell route, with certain demon commanders or lords leading larger warbands, even warbands hostile to each other.  The entrances to which are scattered around hell, and the only way to breech into these 'lower' realms is on certain random tiles in hell.

This game simulates the concept of deities, so demi gods, demi deities etc might be an option also. You've dug down, but have you built up far enough ;). Minecraft uses a portal system which is reasonable but a bit magical perhaps for DF. After a certain amount of religious items, a lot, have been constructed, (some parts found in hell?), then evil and good dieties can show up at sacrifices or services. With the risk and relations that can then come of it, much like regular civilisations only much rarer that they interact at all.

Sort of a post endgame moment. Beyond deities you have zone lords or time lords who can manipulate the fabric of the fort, rather than just its inhabitants, etc, though that sounds harder to deal with. It depends how much challenge you want in the game.

Maybe we should have the current demons that pour out as lesser demons and have demons as powerful as deities that command them (think Malbolgia in Spawn). However, they are always trapped within hell - until you let them out.

Why not just have more demons?

I liked the idea of demon mega beasts. Now you will know what demons fear sort of thing. Plus if it's a huge multiple tile worm that can dig through almost everything except for smooth metal walls. It could hurl magma.

It could also lay a single single tile egg that would act like a wall, but left long enough would create a new worm.

Once you get your Magma Creating Worm Cage of Gold for Unlimited Magma set up, you could have something else.

Perhaps a bigger worm? A worm man? The devil? A worm in the shape of a devil?

Hey I have an idea. Hell could have simply more dorfs. Because dwarves are truly the most fearsome creature in the world.

Why not have all of those things. Demonic mega beasts that will do battle with you alongside the lesser demons, commanded by deity-like monstrosities. Perhaps the deity-like creatures don't actually exist in hell itself in a physical form - they merely lord over it and are worshipped by the coarse, lesser monsters.

Maybe the demons that control civilisations at this moment escaped from the underworld so that they could escape the control of the tyrants, or perhaps work for them as their spies. It would be interesting if you could, depending on the motives of the demon in question, work together with them against the demonic hordes so that the civ-controller does not fall under the tyrants' influence again, where he will almost certainly be punished for all eternity for defying them.

Remember that Demons are currently "Powers" meaning that as of right now they can fool people into believing they are dieties.

Megabeasts however being closer to Incarnate gods (and some are such as the Titans) and are even larger then demons.

A Megabeast Demon would likely be a legitimate Demon God. Since I usually say that gods are somewhere around Megabeasts in terms of strength.

Hmm think megabeast caves in hell, only they would be small palaces or fortresses themselves perhaps? Lairs or some such, where these demon lords or demi demon gods reside. Fallen Demi Gods or Fallen Gods ;).

Another option is they actually have to be opened, tombs as it were.
