This has happened to me a few times. I'll notices things aren't getting done and then find out that my first level of up/down stairs has turned into just down stairs? Did I inadvertently do this somehow? Is there anyway to fix this other than digging more stairs?

Were you deramping anything?  The remove ramps/up stairs designation will also turn up/down stairs into just down stairs, so if you were doing anything with removing ramps you might have accidentally selected your stairs as well (I've done it a few times too).  Or maybe you accidentally hit z while designating something else.

You can fix it by using b->c to rebuild your up/down stairs, they'll go on top of down stairs and work just fine.

You problably mined into your stairs by accident. You can build upstairs ontop of downstairs.
