For the AoO bullrush thing discussed ingame:

Assume we have two fighters with 18 Dex, the Combat Reflexes feat but not the Improved Bullrush feat. Let's call them Fred and Tony. Each can make 5 AoOs per round.

Fred decides that today would be a good day to bullrush Tony. Since Fred doesn't have Improved Bullrush, this provokes.

Tony decides that, for his AoO, he wants to bullrush Fred right back. Since Tony doesn't have Improved Bullrush, this provokes.

Fred decides that, for HIS AoO, he wants to bullrush Tony again. Since Fred still doesn't have Improved Bullrush, this provokes AGAIN.

Since this is a new opportunity (as per Combat Reflexes and Additional Attacks of Opportunity, "[Combat Reflexes] does not let you make more than one attack for a given opportunity, but if the same opponent provokes two attacks of opportunity from you, you could make two separate attacks of opportunity."), Tony gets to make ANOTHER AoO for the new opportunity. He uses this to bullrush Fred. And so on and so forth until both get all 5 of their AoOs for the round in. Or decide to stop, if they were wusses for some reason.

Then (and only then) do the opposed strength checks to move each other start happening. Those are resolved in reverse order from when they are declared, starting with the most recent and moving backwards, not stopping even if one or more of the combatants end up prone or too far away to make a bull rush normally; the fact that they were in range when the bullrush was declared is enough for them to perform it.

Similar results are had with Grapples, Disarms, Overrun attempts, Sundering, Trip attacks and anything else that you can do as an attack action which provokes.
