A murderous ghost that you cannot memorialize for some reason. It'll just periodically kill off your dwarves and there's nothing you can do about it until the ghost dies of old age.

Sums up the hilarity of Dwarf Fortress's bugs pretty nicely.

I'm going to say forgotten beasts made out of high-strength metals (bronze, iron and steel). They're basically bronze collosi, so nigh-unkillable, but with extra optional syndromes, and they can''t just be caught in a small wooden cage. And some of them can fly.

So yeah, if anyone ever gets a steel raven with deadly dust...

Worse? A unslabbable vampire murderous ghost. Now this ground is forever tainted. You can't ever reclaim, cause it will still be there.

Murderous, immemoriable, vampiric or necromantic ghosts. My list contains more common foes, though.

Without modding: thralled giant sponges.
With modding: Let's say you make giant sponges a civilization (capable of thought and movement I guess) that trades. Let's say you accidentally crush an insane giant sponge merchant under a bridge. Giant sponge ghost. GIANT SPONGE GHOST

Don't screw with anything that has both hooves and horns/tusks. Both of these things do terrible things to dwarves.

They also tend to be large. A dog-sized uniphant wouldn't be scary at all.

The absolute worst creature one could possibly get would probably be a slade spider capable of flight and with deadly dust.  How so you might ask?
