Postal 2, especially with the weapons from Apocalypse Weekend (there's a mod that combines the base and addon for best results). From dismemberment through lighting on fire to use of chemical warfare that makes people vomit blood. It's pretty insane.

I guess Hitman: Blood Money can be violent with some of its kills. Like from what I recall, some of the kills using tools were pretty violent. And civilians do run around in terror.

Less predatory, but very violent is The Punisher, where there's a lot of unique interrogation methods that can turn excessively violent. Not to mention the quick kills in general. Though there's no civilians in it (at least not ones you can murder).

Well, if Hatemoo or Hellmoo were still good, playing an abomination, vampire or zombie pretty much puts you in the role of a absurdly violent predator, with abomination being the peak of that experience, but also the most game-changing and dangerous. Too bad both went to shit, I've been searching for MUD/MOOs that also let your character and the gameplay change in drastic ways depending on your choices, like hellmoo did, but no such luck so far :C
