In order to state the obvious.

They are poor, because they smoke. Due to the fact that stuff is adicting, it screws up human priorities.

Also, if you cut obesity, you cut away 30% of America's population. Besides, overweight issues are often caused because fast food is the cheapest.

Yeah, obesity and related problems tends to be a problem for poorer people because fast food is both available and cheap. A lot of cities have whole neighborhoods with essentially zero grocery stores. You can't really eat healthful food when you're buying from gas stations and McDonalds.

I don't really think it'd be that common for a McDonalds to open somewhere where the population was small enough that there were no shops. That would be a business decision only slightly above that of not opening a shop that sells food in a place that apparently needs some sort of grocer's shop.

If you want to be pedantic, "healthy" refers to an individual being in good health, while "healthful" refers to something contributing to someone's health.

Generally, "healthy" is used for both now, but "healthful" is still a legitimate word.

I wouldn't want to play... any game running.
Arma would be pretty much impossible with the jerky hand movements. Or any flight sim.
