Looting OCD. LEAVE NO STONE UNTURNED! Also, steal everything not bolted down worth more than 3 dollars/septims/gold.

Tetris disorder. In games like Dungeon Keeper, DF, Evil Genius etc. I'll build rooms preparing for a large base instead of a smaller one like I have now, even if it means wasting more money and time. This tends to, somehow, produce movement times longer than it should be.

Ammo hoarding, yes. Buried in piles of ammo that I may never use. It wasn't bad on F3, ammo being weightless and all, but it sucks for games like Rogue Survivor and every other game where ammo weighs something or takes up an inventory slot. In F3, I hoarded guns, too. You should've seen some parts of my Megaton house. On the second floor, the walkway where the Nuka-Cola machine is? I PILED guns in the space next to it, where the table is. Dozens and dozens of guns of all types, sizes and conditions.

Looting mostly, though I also have a bit of Ammo Hoarder. Sure, I could open fire on that Big Daddy with my shotugn, but I may need those shells later. Never mind that I have max ammo for it, it's still wrench time!

Unfortunately this bites me hard in games that have realistic reloading where the clip is ejected if you reload.  Haha!

I wonder how you'd react to Red Orchestra then... Removed clips are kept, only you may find yourself loading in a new clip for your LMG with only two bullets in it later <.<

Or RO2, where bolt-action rifles are made more realistically then in most cases, where if you've bolted the weapon and load, one bullet is gone.

I must reload whenever there's a break in the action, even if I only used one bullet.

I will never use consumable items of any kind.  I might need them later.  This is crippling in roguelikes, I'll die with 100+ charges for half the wands in the game and ten different potions sitting in my inventory.

If I stop playing a game for more than a few days I'll be unable to bring myself to start playing it again.

I satisy quite a few of these, but the main one wasn't on the list, so I chose the saving/loading one.

I can't stand anything permanent happening to the game environment that is in anyway suboptimal. I don't mind so much bad things happening if I can undo them, but anything which leaves a permanent mark, even only a slight one, really really irritates me.

Take for example DF. I plan out my forts ages in advance and I don't mine out a single tile unless I know I want it to be like that in the final product. I don't care if it'll save me a load of time, or let me get my irrigation system set up in one year instead of five, I will refuse to vary from the plan. I fully realize that I could mine out a wall that the plan dictates should stay smoothed and replace it with a constructed one that looks the same and functions to same, but I find the idea of doing so so abhorrent.

If I'm playing an empire building game (for example, right now it's age of wonders) and I'm on a scenario with AI-controlled allies, and the allies are about to be defeated, I will usually gift them a base behind my own lines. I don't care that they're not going to be any use in the future, and it'll be hurting my own economy a bit by doing so, but that stuff is just temporary and if they get wiped out it's permanent.

Gonna reset the poll and expand choices to select up to 5.

EDIT:
I guess I can't change the # of vote selects. Resetting nonetheless.

For Gaming Masochism, I've started using Hard as my default difficulty instead of Normal. After all of these years, I've just gotten too good at this stuff to be challenged by Normal anymore.

I don't know if this would be Looting or Ammo Hoarder. After realizing that I could craft ammunition from junk and create homemade Nuka Cola and Sunset Sarsaparilla in New Vegas, I began hoarding all unbent tin cans and pop bottles I could find.

I also take just about everything worth more than half a cap. I hate carry limits so much that I normally cheat the carryweight up to insane levels immediately so I never have to worry.

Actually, come to think of it I know a guy that does this.  I always tell him that he is just spoiling the game this way and he might as well just go watch a movie or something instead.

That's usually why I try to find safehouses as early as possible, even if it means being stuck at walking pace for an hour or few. A funny way to troll the game is if I have just over 200 weight (204-206), drop an armor, and carry it manually (X on keyboard), and have just under 200 weight in my backpack, speeding up my time to get to the safehouse. All garbage items still in stowaway, and an armor in my hands; somehow, not slowed down by over-encumbrance.

U mad game?

EDIT:
In regards to FAQs, myself: As of right now, I'm managing my loot in FO:NV to see what I can use, and what I can sell as garbage, kudos to the wiki. My barter skill ain't much, but every cap counts. As long as it's something I won't miss, I'm okay with selling it. I've also used the wiki only to max out my startup (most useful SPECIAL and skills/perks), and all the Goodsprings tasks and good loot as well as maxing out XP on start. And only use the guide as a selling and crafting cheet sheet (again, what's useful, and what's garbage, until I have no need to do so anymore).

I miss the stuff you can build and use in FO3 however. I miss my dart gun and railway rifle. :'(

What about those of us that just cannot finish some games? I'm looking so sadly at you, Shadow of Chernobyl. Doesn't matter that I've reset three times, playing through once with only sidearms on Master difficulty. I just can't bring myself to finish it or a few other titles.
Just added it: SEE- Incompletionist.

All of the above (the reloading bit is especially bad in WoT with the French tanks, where loading times can go up to a minute for a six round magazine, and I fired a single shot, and I must reload, every, time).

Also the symmetry thing. If I can, I have an even number of each shell type, I build even numbers of different units, bases need to be perfectly built to an ever changing template.

My main OCD has to be the "Magellen-Minded Disorder". If a game has a map that is filled as you visit areas, then I must visit them. Currently trying to do this on Morrowind with every single cell, since I can't stand that there may be a single brown square on my map.

Looting everything and quicksaving all the time (despite the fact that I rarely, if ever, reload) are also big ones.
