I know this isn't a computer game, but the tabletop RPG GURPS is pretty lethal, especially in (post)modern games.

Or in high magic games.  The game I'm in has a phoenix that does something like 4d6 damage with a firebreath-like attack.
Or the Murp-cannon that does 5d6.

I know this isn't a computer game, but the tabletop RPG GURPS is pretty lethal, especially in (post)modern games. The weakest modern pistol, an automatic .40, does 2d6 damage, may cause bleeding if those rules are in play, and can fire up to three rounds per second. This is in a world where the average Joe has 10 hit points.

A limb/extremity hit might not kill, but a roll of 3 will cripple a hand/foot, a roll of 4 will cripple an arm/leg, and just one point of damage will cripple an eye. Four points of damage to the skull will result in unconsciousness, heavy bleeding, and eventual death.

Double the damage values above and you'll irrevocably render that body part nonfunctional.

Healing isn't a joke either. An average character has a 50% chance of healing one hit point each day.

Nothing wrong with that, if people want to mention some tabletop games feel free. I'm not versed in the subject, but I heard Dark Harvest is quite lethal.

Even mundane stone age combat in GURPS can be fast and brutal (for the characters at least). An average person with a spear does 1d6 points of damage (1d6+1 if using both hands to wield it) and any damage that gets through armor is doubled. Most characters can't die untill they hit -10HP though, they just have a good chance of passing out.

The best defence in a gun fight is cover. Lots of thick cover.

The Lone Wolf CYOA book series from the 1990s is pretty deadly. You can die at any point by making a wrong decision or a risky gamble. It's conveniently in computerized form now, too.

Diablo 3 hardcore mode. Not so much because you can die, that's the point of the mode. But because you lose all your carefully harvested gear as well. It actually brings a smile to my face that some people might have paid actual money for some of that equipment. Funny stuff, especially with 1HKOs in inferno difficulty. There's some horror stories waiting to happen.
Is this hard because the game is actually hard, or because people have buggy internet and can't dodge lagged attacks? :P

For the thread, I'd say God Hand is pretty dang lethal. You do better, it gets harder. (mostly involving dodging, if you get hit it goes back down in difficulty slightly) You get to Lv. Four Die in a level, say goodbye to your health if you get hit. Bosses on Lv.Die are probably horrendous.

A lot of enemies develop one hit kills at that level too (they don't common use them but I have experienced one). Though I will say that the game is a lot less hard one you understand what you are doing. What is hard are enemies that break the pattern of combat.

Also I missed the Combat part... Sierra point and clicks don't generally have combat (Well ok... Quest for Glory does and yeah some of its combat is pretty lethal but it isn't what the thread creator means).

Bushido Blade 1 and Bushido Blade 2 are fighting games BASED around lethal combat. They are almost all one hit kill based. No squads though.
