One thing worth noting here is this is state's opting to suffer. The Federal government was offering 100% of the costs of the Medicaid expansion for the first two years (2014-16) and 90% out to 2020. State's that make an ideological stand are putting that ideology over the healthcare of their residents very explicitly. I'm not sure that's a winning political strategy.

That is almost EU-like thinking... "Here's free money, as long as you join us and our ways. Welcome to the fold".

Depends on your perspective. European governments don't order their citizens to run out to a private insurance agency and get coverage. So, to me, it's not really anything like what European countries do.

Compulsory insurance applies to those below a set income level and is provided through private non-profit "sickness funds" at common rates for all members, and is paid for with joint employer-employee contributions. Provider compensation rates are negotiated in complex corporatist social bargaining among specified autonomously organized interest groups (e.g. physicians' associations) at the level of federal states (Länder). The sickness funds are mandated to provide a wide range of coverages and cannot refuse membership or otherwise discriminate on an actuarial basis. Small numbers of persons are covered by tax-funded government employee insurance or social welfare insurance. Persons with incomes above the prescribed compulsory insurance level may opt into the sickness fund system, which a majority do, or purchase private insurance. Private supplementary insurance to the sickness funds of various sorts is available.

Glad health care law was upheld (and in the way I'd hoped it to be upheld, other than the way that would have pissed me off) and the blatantly unconstitutional medicare thing was struck down. Perhaps there is some sort of justice left in the US.

This is terrible, and it's really simple to see how it's unconstitutional.  Unless a power is explicitly given to the federal government in the constitution, it's a state-power.  I hate government overreach so much.  It doesn't matter if it helps save lives or not, government shouldn't force us to buy something or face a penalty.  If we had actually consulted physicians and doctors on what would help Americans I wouldn't have such a big problem though.

The Constitution says what it says. More specifically, it gives power to the SCOTUS to determine whether a law follows the rest of the Constitution. The SCOTUS found it constitutional.

(I'm not a huge fan of the government pressuring states with money to get them to act the way the government wants, and I'd like to see it end across the board, but small victories.)

Edit: I'm all for a two-tier healthcare system, with a basic government provided plan, and a more specialized private plan. Besides, America does quite a lot when it comes to medical R&D for the world, thanks to a competitive healthcare market. I just get a bent out of shape when people say healthcare completely provided by the government is a solution.
