I don't use therapist, but I do use autolabor, which performs a similar function, but automatically.

I use stonesense occasionally when I want to take a look at how my fort is doing, but that's about it.

I use Dwarf Therapist and a tileset; they just make the game so much more accesible. (I don't use any of the features of Dwarf Therapist that would circumvent otherwise uncontrollable units, i.e. children)

Why not just use the ingame menu to toggle off cooking for these?

I enjoy playing vanilla DF, but actually literally(not in the non-literal usage) get migranes from the harsh color contrast on new monitors, so i make do with a muted tileset most of the time.  Having been around DF for awhile I can't express enough how useful it is to know how to play the game vanilla.  I've seen far too many utilities discontinued to want to be reliant on any of them.

Letting go of the micro manage lets you enjoy the haphazardness of the game.  But then i'm the type who enjoys just having an open hallway into the fort for an entrance; I can't remember the last time I built a trap in DF.  That said, I know some people play the game to have hyper efficient, impregnable cubes.  To each his own.

Every new version I run a vanilla fortress or two. Then I start modding.

The only things I change is the pet burial thing and water depth displayed as numbers in the init file, but I still consider it 'pure' DF.

Never used dorf therapist, mods or tilesets in all the years Ive been playing.
I think toadys confusing interface better simulates the experience of a bunch of disorganised drunken dwarves running a fort.

I only play 'pure' DF in those small intervals when a new release comes out and the tools are unavailable. Those are dark times, indeed.

this, i really don't see the appeal of wasting time looking around for dwarfs then more time to tweak jobs
i know it's dorfy in the way of maximum effort for minimum gain, but i rather play the game

SO SIMPLE.

add the fact that you have to look up skills and see which dwarf it is
then x that by the number of dwarfs you have.
Not a problem when playing a 20 man 2x2 embark. Managing over 100 dwarfs while remembering all their skills is just tiresome.
Not to mention that if you have an emergency and have to set up temporary labors, you have to spend around 15 minutes before and after dealing with the problem, which totally ruins the immersion of the game. Maybe some people like mandatory manual repetitive jobs, I certainly do not.

I only use therapist to control the jobs and make sure there is only 1 dwarf doing something that needs specialization. 1 Weaponsmith, 1 Armorsmith, 1 beekeeper, etc

Everything else is vanilla

But that's the thing, it's not as hard as you're making it out to be. You only have to change the labours on about 10 dwarves at most at any time. If you're having to do all 100 or so at once then I;m not even sure what you're doing.

I couldn't live without the Therapist.

The biggest use is when migrants come -- I can sort by wave to see all the new recruits, make sure their labors match what I need and don't need at the moment and I don't need to keep track of who is currently labored with what.  I find it easier to determine whether my part-time butcher should be taken off that duty because a new dwarf is able to manage that work now or whether it's better for the new dwarf to be a weaponsmith.  I don't have to go find the current part-time weaponsmith in the fort and see whether his skill level is high enough that he should be moved to full time or whether it's worth it to keep him doing whatever his day job was because the Therapist makes that easy to find out.

I can easily create whole herds of engravers when I want a new dining hall smoothed out then revert them to their previous professions easily.

I also use color modifications, a square 9x9 or 8x8 tileset and a bunch of init/d_init changes.
