I used to use therapist until I realised that you could in fact change jobs ingame. Then I stopped using therapist because it was not necessary anymore.

Sometimes I play semi-pure, just took out the aquifers and changed the init/d_init.
I used to use Dwarf Therapist, but not too much anymore. Even after 100 dwarves, I have no problem keeping track of them.
Other times I play Masterwork, and I need dwarf therapist for that to check out the castes and whatnot. Plus the DFhack clean all command comes in handy a lot.

I went through a phase of playing just Pure but currently I play Genesis  mod in 31.25 as I think it has a good balance between Fort and adventure mode.

I play the new version of DF Vanilla for the adventure mode but I use the wanderers friend mod as I love crafting and making and covering my oustider in self made Bone/ leather armour.

I use Therapist (I don't know how you guys do without it, it's possible but doesn't seem worth the trouble haha), DFhack just to prospect on embark and clean (got to keep your FPS up when you have a couple hundred dorfs), and a tileset but I really haven't changed the game itself in any way. Basically just use the extras that help "polish" the game up a bit.

"Not use therapist."

That's why I said I used to play pure and that I now consider therapist a must.

But therapist isn't a must at all!

Of course, but that only works for individual dwarfs.
The thing with Therapist is that it gives you all that information for every dwarf without having to track them down and displays it all in a very neat color-coded and very intuitive chart with detailed skill values.
Makes fort optimization and job sinergy so much more accesible and makes managing 150+ dwarfs not give you headaches.

For me at least.

You must be able to keep track of more information in your head than me then, I guess I'm quite a visual person.

I play a completely vanilia game.  I've used Therapist in the past, but I usually prefer to set migrant labors as they come in and forget about it.  I like the strategic element of deciding how to use a dwarf when it shows up.  I'll sometimes tweak labors upon a mass die-off, if, for instance, I don't have any miners or farmers left.

I use stonesense and overseer to take screenshots of my fortresses, but I've never considered using them to actually play the game.  I think I prefer the pictures in my head.  I don't use tile sets for the same reason.  I visualize a world that looks a lot different than the ASCII, but I think I need something as abstract as ASCII to kick it off.

I modify the raws from time to time to fix blatant bugs.  The cat-grasp spam from 40d and starving kobolds for instance.  I've modded in some reactions in the past, but I actually prefer the limits of the vanilia setup.

Pure except for init and d_init settings, as well as adding thongs to entity_default for dwarves.

...SHUT UP, it's for, uh, clothing diversity!

I play 'pure' DF although I change some init settings. I don't add anything or tinker with raws though.

Oh, yeah, therapist. Although really, that's not changing the game, just taking tedium out of it.

My actual DF version is 99.99% pure.

That remaining 00.01% is custom color palette.
