"+" is a simple hit recommendation, saying "this is the most likely attack to hit and do good damage right now". "-" is a warning, saying that "this is below-average in terms of chance to hit and damage". The lighter the plus, the better. "!" means that it's an attack of opportunity, that one of your choices under that attack will be better than the rest, such as left foot kick or stab.

Ah those lovely times you get a blue "!" for your main weapons best attack... On the targets head :D

Repeating yourself again while totally missing the question makes it pretty impossible - to chill. If you're suggesting those desccriptive terms have anything to do with the colored +/- symbols, then you haven't been paying attention to them.
Impossible hit you say... I say it's simple !

Indeed, I don't believe in luck. All skill, you make your own chances.

I think if this part was true it would be both consistent and redundant. The descriptions already say which attacks are most accurate and most damaging, but the symbols only match the descriptions and the actual results coincidentally.

Maybe it's an indicator of whether a successful attack will deal negative status effects.

Since it shows up on tongue attacks, I highly doubt that.

The pluses and minuses are relative--as in, if all your attacks are impossible, can only graze and one is difficult, not square then the latter will have a +.

In practice, not consistently, and like I said, if it were, it would be redundant. I've seen + next to poorer strikes in both accuracy and effectiveness next to clear hits with no notation. I've seen - next to attacks that look no worse than similar, which trickier or less damaging hits have pos to brighter green pos next to them. Your suggestion falls apart under more than cursory observation.
