This was back before 40d, back when the "screwpump water duplication" bug still existed.

I was attempting a DIY purpetual motion machine with water wheel powered screwpumps.

Replicated water gushed out of the reactor, and flooded the whole map.

I once had a magma crab escape the volcano and occupy my general stockpile. It managed to kill half my fort before an unarmed recruit bit it to death. Had I simply remembered to refill the hole after mining some gold ore it would never have happened.

In a late mangled fort that had no ores at all, my military was all dead, and had a huge siege of gobbos loitering outside, so i couldent get wood. I couldn't breach the caverns as I had no military, however I did have a huge complex magma pipeline system from a volcano. Anyway, a caravan finally managed to get in, and since I had no ores, it was the only way for me to get weapons. I bought 2 swords and got a guy I planned on having duel-wielding them. I really wanted to turn said guy into a complete badass, so I got a "disarmed" thief and put him in a cage in a small room. I did not even lock the dooe as I assumed my guy would pulverize him. Turns out that the thief wasint disarmed, and I accidentally set my "baddasa"es squad uniform to archer. And I had no ranged weapons. The thief, after taking a few fists to the right lower leg, took off my "badass"es head in a single stroke. I then look around my empty basement to see if anyone saw me. I pulled the lever to block off the floor that the thief was in with a drawbridge, then the lever to flood the floor with lava. Well, the lockdown lever worked fine, but instead of pulling the laver to flood that 1 floor, I pulled the incomplete "magma all" lever. And guess what floor was incomplete? The one the gobbo was on -.-.

Learning that in spite of not being a walkable tile, a well does not stop the flow of water. Entire fortress a dead loss.

Oh, and have I mentioned that an elven caravan turned up right as I designated a ramp to be dug into the only unflooded part of the fortress, namely the garbage dump?

Never used Dwarf Therapist, mods or tilesets in all the years I've been playing.
I think Toady's confusing interface better simulates the experience of a bunch of disorganised drunken dwarves running a fort.
