Something scary happened in my fort

My favorite dwarf died.
I had been afk when it happened, so I didn't see the message. I only knew because she was the mayor/countess/broker, and there was a new mayor and no countess. Also, her dead body was inside a coffin.
I looked everywhere, trying to see if some kind of invisible killer had snuck into the fortress.
After a while, I checked in the gamelog file.
The killer:
Old Age.
What am I doing wrong?

It doesn't sound like you did anything wrong, aside from not killing the noble sooner. Old dwarves do tend to become mayor, and if you appointed your mayor baroness, that would mean your countess was old, too. Dwarves live 150-170 years before succumbing to time, and sometimes they come to the fort at age 140+. Since old dwarves have had a lot of time to accrue skills, especially social skills, they get elected mayor often and are seemingly-nice for their job when they migrate, so that explains why you decided that dwarf was good and chose her as a favorite.

You'll never get a new countess, by the way.

Never. Sorry. I always make sure that my counts/countess is a grandparent.

Yes, the same as any other dwarf can.

...Not that being a parent, or grandparent, actually helps, since if your baron/count/duke/baroness/countess/duchess dies, you do not get another one. Corai is simply mistaken in his belief otherwise. The succession tags are nonfunctional after world generation is complete, thus if you lose your countess, you do not get another one under any circumstance.

That sucks.
Still, her firstborn son liked musical instruments, whereas she liked grieves and shields, so I suppose no count(ess) is better than a bad one.
Besides, I've still got the queen to deal with. *Shudder*
She keeps banning the export of trumpets, crowns, and scepters.
Worse yet, she mandated I make large gems.
