I don't believe ghosts normally construct things, but the creature was probably an unnamed war animal, so the game couldn't be bothered to remember it.

The dwarf made the box, then died and became a ghost. Names like that are applied retroactively.

The creature that destroyed it is unknown because it isn't an important historical figure. Probably a troll or something that never got any kills and thus never attained a name.
