I just searched three entire 16x16 regions and did not find a single demonic fortress. Am I just unlucky?

There should be at least 1 present in every 16x16 area somewhere. It's possible that your world might be bugged somewhere.

I've had one so far. Normally I know something is up because of the FPS drop, but this time I was into my second spring and setting up my spike hall when I noticed an extra upright weapon in the list.

I haven't been deep enough to see it yet. But I know it's there. This is actually the first time i've found one. I zoomed on it an it shot me down soooo many z levels just to show me blackness. So yes they are in this version. Assuming we are talking about the HFS fun stick that ushers in a rain of clowns.

i thought these were restricted to mountain biomes?  Guess I was wrong.

While adding magma to anything will make it dwarfy, adding the word "magma" to your post does not necessarily make it funny.

I've seen 3 in mountain biomes, they are certainly more common there.

Nah, Just started a new fort in a hybrid Mirthful forest/Good desert.

There's a longsword just sitting there. Waiting.

I had one that murdered my FPS without even being opened by me.  It had an opening into a cavern which I breached, and although I sealed off my staircase their perpetual clownish pathfinding attempts made it impossible to play that fortress for very long.

I came across one by "Accident" in a mountain fort I originally built purely to make loot for an adventurer.

There really isn't anythign different about them from earlier versions.  As of now they Zombies and "fun" wander around as soon as you come across the Happy Fun Fort.  usually by a year it is totally deserted.

Do you have to have multiple cavern layers to have one?

I tried creating a lever but it said there was nothing to be hooked up, I tried looking at the weapon stocks and it wasn't there, and I searched manually. Nothing.

I'm not sure if it's a bug or not, but if you have less than three cavern layers, you never get a curious structure.

With that said, there's no other reason to have three cavern layers, since they are just FPS sinks, anyway.
