I think there is something else going on.  I have noticed in 0.34_5 that occasionally my Trade Depot will suddenly decontruct.  I haven't noticed this happening with grates, but then again I do not tend to use a lot of grates.  I have also noticed that my wells will also suddenly deconstruct - all of them at once, like 5 or 6.
Personally, I think this is a bug of some sort as I know that I have not done anything that would cause this on my own, nor have I had tantrumming dwarves either.

No, plain vanilla.  No tilesets either.

I'd have to check later tonight, but what I remember is that part of the map is evil and the rest is neutral (forested wilderness of some sort).

Maybe it's a new bug/feature of evil embarks. This calls for science! Rebuild the things and keep track of which biome they are in, along with any weather or units may have migrated on the map. And keep some guard animals near the affected areas to detect possible invisible enemies.

So I checked this game last night:  it is an evil biome on the far left that rains blue malodorous ooze (causes blisters and vomiting).  The top-middle of the map is a non-evil forested region while the middle-right of the map is a non-evil black sand area.  I haven't had any evil undead show up (except for vampires, who are in their own external barracks building) so no problems there either.

The 5-6 wells are underground in black sand, located very close to my underground farms.  There is a wall and a pair of doors limiting access to the wells and nothing else in there except for black sand.  This area is only used for washing up since the water is stagnant.

There is no way that this was any sort of mass dumping or decontructing.  #1, there is only black sand in this area (and the wells) and no stone, so there was nothing to mass dump.  #2, the wells are sufficiently far from any other stone source too, since the farm is large and entirely on black sand as well.  #3, the water for the wells is fed from a stagnant pond above ground, which has to pass through a fortification and a floodgate first, so there isn't any way that an enemy passed into the water supply and caused this to occur.  The fortification and the floodgate are fine, only the wells deconstruct.

As I said, the wells are stagnant water and are not used for drinking - that's what booze is for.  The wells are used for washing off the blue malodorous ooze and this tends to pool up around the wells quite a bit.  I have no idea if this is relevant or not but thought I would throw that out there.

You can mass designate items to be dumped with the (d) (b) (d) tool by accident, without ever touching the stocks screen...

In more recent versions I've noticed that there no longer seems to be a notification of building destroyers' actions. So if a troll, for example, breaks into your fort through the caves you will no longer know until he's halfway up the stairs. I do not know if this is still the case if a dwarf is around to witness the destruction.

Nevertheless this lends credibility to the  idea that perhaps you've got building destroyers running around.

I definitely do not have building destroyers running around inside of my fort.  Until very late last night I had not even punctured the first cavern layer, and I only had my first siege last night as well.  It would be weird for an invisible building destroyer to smack out the 5-6 wells but leave the doors alone (which by the way do not show the "taken by an invader" text).  To get to the wells would require traveling down the main traffic corridor to the main traffic stairs, through the farming/brewing/drink storage area, etc.  Urist would have noticed somewhere along that line.

I have not used the stocks screen to do any mass changing of dumping, melting, forbidding, or whatnot.  It is most definitely not mass dumping, regardless of the form or method to do the dumping.  Next time it happens I'll try to post a save or log or what-have-you.  Might be a while since it happens so infrequently.

It's far fetched, but any flying or ambushed creature could take down all of the grates, escape without being noticed (and wouldn't give the taken by invaders line), wouldn't destroy the door if it was blocking their path...

And things don't get dump designations without player interaction, so you must've done something by accident
