@op: I would add more spreadsheets, less graphics, more multi-player opportunities, and then not change anything for 2 years.

I think using the older Ultima games for influence would generally be a good route.  I remember Ultima IV was massive, lot's to do, NPC's had lives and routines, there was an ultimate quest but it was still very sand-box'ish.

Specifically right now though for me, medical treatment and better inventory management.

2) party management, aka giving your doods simple orders like "go there," "kill that," and "leave that groundhog alone."
Liked all of the suggestions in your list, but especially this. The party AI & control is terrible (chasing random creatures, bowmen who can't reload, etc.). This wouldn't be a big deal if your party wasn't so necessary for keeping the bogeymen away and helping you survive your first few quests. Giving them orders (eg: reload, stay put, pick up better weapons/armor and use 'em, sneak attack) would help this experience a lot.

Definitely making a realistic economy & a world that (even slightly) changes/adapts as you interact with it would be great.

Funny how 95% of what is being requested here is on the TODO list.

Dwarf Fortress will be a flawless game when completed indeed.

Medical care and follower controls (along with means of managing follower inventory) are probably the features I'm waiting for the most.

Then the time passing/worldgen continuing sort of deal.

Closely followed by the housebuilding/landownership stuff.

That said, I'm fully aware what all Toady has planned.

I'm looking forward to being able to have a family, build a farm, and do some timeskipping along with generally sims/farmville nonsense when I feel like taking a break from adventuring for a while, but as awesome as that will be, they're longterm goals and not exactly a priority for better playability for now.

It would be awesome if they made it so you retire from retirement for a reason.
Like warebeasts kill your family (and you start with a quest to kill them) or goblins kidnap them and you have to go and rescue them. Even just getting divorced because you are broke and have to head back out with the sword to mine some money out of some skulls.

I REALLY hope we don't have "your spouse was stuffed in a fridge" as a standard part of adventurer backstories. I mean, I know we're supposed to have some cliches and all, but come on...

"The money ran out" or "I felt like killing things again" are much better excuses.

Besides, why can't adventurers adventure while married?

Shops selling steel equipment, i dont care how much, i already have got thousand gold coins, and probably more while having a lot of faint diamonds from last quest.
As human i can only get steel+ weapon, unless dwarf is very luck and makes human sized artifact.

Formations. Party orders.

Medical treatment. My adventurer has rot everywhere right now.

Friendlier UI (less commands = better)

Dorf cities. Humans are boring. I'm surprised Toady didn't work on mountainhomes first, but it makes sense. Dorfs don't like to build sprawling civilizations.

We play a game based around hapless villagers asking you to save you from everything in a world, and then after a while a lord of a castle/Fortress/Priestess sends you on more important holy quests.
And you are complaining about Clichés?

I want to be able to do most of the stuff possible in Fortress Mode. Digging(not just soil/sand), cutting trees, contructions, crafting, etc. I also want artifacts to be created in world gen, maybe with mystical magical properties. So for the self-imposed quest of finding a lost artifact ring that is said to grant  strength and endurance I would dig through a whole area, excavating ruins, tunneling mountains. In the end only to realise that all this work already made me superhumanly strong, having no need for the ring anymore.
