I think of DF "stairs" as "ladders" instead.

It makes a lot more sense, once you think of them that way.

Think emergency stairwells in large office buildings; thats what DF up-down stairs are. ;)

Myself, I use ramps for main hallways, and stairs as needed for "maintenance" access ways, or for the upper/lower housing level in my 3-z level 30-room bedroom blocks.

I really don't care, though I am leaning toward ramps to reduce lag and make my floors separated by far too many traps more logical.

I figure up/down stairs are a spiral staircase, which can be quite small, but also cramped. Its very space efficient but I generally only use them for maintenance passageways, for things that are accessed only very rarely and by a small number of dwarves.

I prefer the main entrance of the fortress to be done with ramps. This even has tactical benefits, in that dwarves can see all the way up or down the ramps, so if there's a threat they won't blunder into it like they do with stairs.

Inside the fortress I do use a lot of 2x2 blocks of up/down stairs, but I figure them being just a larger spiral staircase.

3x3 staircases bother me like building an entire fortress in dirt layers bother me.  Otherwise, I like healthy mix of ramps and stairs, for high and low traffic areas, respectively.

Will a wagon go down a Down ramp that's three wide when there's no open space past it? Kinda something liiike....

IIRC the ramp must be against a solid wall for it to be usable. So you couldn't put it like that against a floor above an open space.. But yes, ramps are more efficient in that (again IIRC), creatures moving over them move both horizontally and vertically at the same time, rather than one than the other as is the case with up/down stairs. They also have the added bonus of allowing you to trap each level of your 'stair'way.
