Designations are the bane of my existence:

"This mining/channeling/building designation is perfectly safe, they couldn't possibly screw it up."
"Okay, this time I'm sure the assignment is safe."
"I can't be arsed channeling this out tile by tile."


Levers are fun, too:

"Pull the lever! ... why isn't it doing anything? Why is it not linked up?"
"Crap, I mis-labelled my levers."
"I think I linked this bridge/gate/trap to the wrong lever ..."

It happened on my first zed siege. When both necromancers spawned, one shot the other in the gust and killed him. He was a dwarf and member of my civ so he became a ghost that ressurected all shit on my fort killing everyone.
