IT'S RIGHT THERE.

I keep getting a message about dwarf so-and-so is unable to place item in tomb, cannot find path. They do this whilst standing in the open. They do this whilst standing next to the tomb. How fix?

Some door to the tomb is forbidden/locked.  Try seeing what doors near the tomb and could be blocking access.

There's probably no path to the item that needs to be placed in the tomb.

Sometimes in the heat of battle, body parts fly up and land on impassable tiles in workshops. You have to deconstruct the workshop to get it out.

First, figure out what dwarf the tomb belongs to and what body parts are already in it, if any.  Then go to 'body parts' and 'corpses' at the bottom of the Stocks screen and look for pieces of the dwarf's body that are lying in odd places.

It was the part where a dwarf's head got knocked off his shoulders onto a 21ft. high wall. Had to scaffold on up there to get it =)
