Worst, Most Unexpected Fortress Deaths

I just lost a fledgling fortress. It started badly: Ten creepers (modded-in Minecraft-inspired monsters), several crocodiles, a sturgeon, a sea lamprey, and four cute ducks. Those weren't the issue, though, the GIANT REPTILES RIPPING MY DWARVES AND DOMESTIC ANIMALS APART and the BIZZARE PLANT-THINGS TURNING THE SURFACE INTO A DWARF BARBEQUE were. I had one survivor, a craftsdwarf, who ran to the pick through a raging fire, then underground, and started digging. You know, I just might--nope, the burn wounds caused the last dwarf to bleed to death, literally just as I was thinking I had hope.

Creepers? I guess they have cave-in-dust nullified syndrome or something to replicate essplosions?

At least, that's what I've done :)

Had an unexplained cave in that compromised my main gate.  Fort entrance was dug into the side of a small hill, and I built walls up the hill and around this area.  Then one day I find the part of the wall that was on the hill over the entrance had somehow caved in through the floor, leaving a nice skylight in the hallway.  I never got a cave in warning, it was just like that when I started the game.  Then the goblins came.

I'm still not sure how this one happened.

Beyond the obvious, I'd have to say one fortress where I tried to keep the river flowing through winter by running magma underneath it; instead it caused it to slllloowwwly freeze and thaw at season changes, with spots of moving magma thawing certain tiles of the river. I didn't even notice this effect until it thawed in such a way as to drop an unsupported floor of ice through the riverbed, magma flow, dining hall, booze stockpile(‼) and dwarven reactor chamber. I sat there puzzling how my power room was randomly filling with magma for a couple minutes before I actually thought to get someone fixing the breach.

I thought my dwarves who just killed a ninja rabbit ice-spitting FB could go on against a legendary-something minotaur...
and a goblin siege came...
my biggest fort was soon reduced to what was goblin bits and 20 scared dwarves because a cat JAMMED THE DOOR OPEN with vermin remains...
but the minotaur took on against the siege :D

Were you the one who posted the creeper mod? If so, yes. I saw a creeper mod, went, "Ooh! Another way to kill my forts too early!" and downloaded, but I forgot to note whose it was.

The fort I bring up the most. I had a very elaborate system of pumps and aquifiers driven by a total misunderstanding of fluid dynamics in Dwarf Fortress.

Dwarves fired out the front door.

Thought I was pitting disarmed gobbo prisoners into my arena to train my brand new military.  Accidentally added my caged minotaur to the list.  Didn't end well.

I'd have to say it's a toss up between the rot to death FB syndrome which wiped out 3/4 of the fort and triggered a tuntrum spiral till fort death or when i First started using pumps and water wheels. Linking a pump to a water wheel with no way to shut it off leads to mass drowning and fps death. I flooded the whole fort
