On a haunted forest map I was digging out some sand caves and after designating some rooms it gave an exception fault and died. It was only about 1-2 months into the game and I had only just built a food stockpile.

If it helps at all there were a metric shitload of phantom spider webs everywhere on the map (but no phantom spiders..), maybe that's what busted it.
