Maybe something was deconstructed in such a way that a mechanism simply dropped off?

I had an invisible mechanism in a magma channel too.  It showed up in the stocks and I could zoom to it, but hitting 'k' and panning over the area showed nothing.  I was able to forbid it with the designation screen by encompassing the whole channel.

This has happened with molten stone objects as well.  I looked at it for a while but couldn't find anything.  Only a save before it actually happens is going to get it unless I get lucky.  The frozen/melt disappear might be different.  I'll create another report for that.
