Twist?

Ill make it quick :)

If i attack someone and my weapon gets lodged into the wound , How do i twist it around inside? (Sometimes enemys do it on me and my dwarfs in dwarf mode do it)

I believe the only way is to step into the enemy(Auto-attack), it always twists if that's available from what I can tell, you can also just pull the weapon out by pressing shift+I and selecting it from the list.

Does the twist actually do a thing? Inflict pain or something?

I think twisting causes a little blood loss and a lot of pain.

Which unfortunately measn there is no way of doing it when your standing ontop of tour foe.
