I can't say for sure, so maybe someone can give me a definitive answer.

Will some dwarves actually cower when sent to attack something? A couple of times, I've noticed a militiaman standing away from the fight while everyone else runs in. I can't say definitively, but I think on occasion, some dwarves will actually go AWOL and when checked in military tab, they are no longer enlisted but still alive.

It's possible that said militiaman is not aware of the enemies (sight blocked by a tree perhaps?). Once enlisted and active in the military, dwarves will lose all sense of fear and charge at any opponent within sight; there is no 'retreat' command.

Like Syrup Roast said, its probably a LoS issue. Ive had military leave the battlefield to get drinks/sleep/eat while there was a squad of bow gobos behind some trees a few squares away.
The best solution to this would be to periodically chop down all trees around your usual battlefield and use them to make ash/charcoal for making soap, glazing statues, and making steel, while giving your wood cutters, soap makers, glazers, haulers, and metalsmiths something more to do.

Hmm, actually it might be possible that some dwarves actually avoids "fighting" , on a few occasions I've had randomly some of the squad members do
anything else than join the fight , but just standing there idle contemplating a nice Statue of Goblin massacres.

I have had soldiers who run away from battle if they are being overwhelmed

I can confirm runners, usually they are green recruits, with no kills who are outnumbered. In that case it either run or trance and many run.

I can confirm that injured dwarves will retreat from the battle, regardless of skill level. Armok knows I've seen it happen often enough.

I'm not entirely certain if dwarves will outright avoid battle, but I have seen squads of three or four recruits switch to "SOLDIER (Will not follow orders)" when ordered to kill something, like, a cave dragon that I am entirely certain they know how to path to.

I've found that ordering them to kill doesn't work quite as well as ordering to move to an area near the target.

Yeah 'k'ill is bugged, stationing and letting them LoS aggro is reliable.
If the dwarf has actually gone from the military screen (no longer enlisted) this might be another bug.  saving 1 fort and loading another can cause dwarves to totally vanish from 'm' menus.
