Building Walls doesn't always create floor above

I'm in the process of constructing a tragedy mule-style tower out of curiosity to see what happens when various things are dropped long distances. Each layer looks something like this:

XXX

With the X's being walls, the . being an empty space, and the ^ and v being ramps up and down, respectively.

What happens is that every once in a while (It's happened twice now and I'm on the 4th floor) the top three walls in the structure don't properly generate a floor tile above them, forcing me to either,

a) Build a temporary floor tile so that my dwarves can get to the center tile, at which point they fall through it and get stuck when removing the floor to replace it with a wall.

or

b) Remove the offending walls and build them again.

This is really weird. Help?

EDIT: In related news, the game froze for a long period of time when a carpenter attempted to move the remains of a mother and child that were crushed during the removal of the third floor, without paying attention to the partially constructed fourth floor. The offending item appears to be a two-humped camel leather shoe. Huh.

EDIT2: It appears to freeze momentarily every time the items are moved to make way for a new wall. Double huh.

EDIT3: Cancel that. It only did it one more time after the first.

EDIT4: Apparently it does do this every time those items are moved out of the way for a wall. I'll stop editing this now.

Aye, I had this exact problem with building walls and removing floors above them. When no one responded to my thread I thought I might've been crazy. Now I at least know I'm not alone. <.<

Wait; someone else had this problem? I'm not alone? Woot! Okay, do you have any method of circumventing it? Cause at the moment I'm a wee bit stuck, as can be seen in the video I posted.

I haven't had any trouble with walls not putting floors above them, but I have had trouble constructing proper fortifications due to fortifications not putting floors above them.  

For instance, if I want to build a tower with multiple levels of fortifications like so:

WFFW
F++F
F++F
WFFW

F= fortification, W = wall, + = floor


Then I have to construct flooring above over a fortification so my workdwarves can reach the corners to erect the corner walls, and then deconstruct the flooring to build the fortifications.  However, since deconstructing a floor over a fortificaton causes the dwarf and the material used in the floor to fall into the fortification below, I have to deconstruct each fortification all the way down the tower under the deconstructed floor above and then reconstruct them afterwards, as the material gets stuck in the fortification.

Really, I don't want fortifications to automatically put floors above them, so they can be used like crenelations on a castle wall. but it would help if the dwarves could deconstruct the flooring from the tile next to it, and leave the flooring material in the tile they were standing on or if they could build walls at diagonals.

Another vote for being able to deconstruct floors/stairs without having to stand on top of them. I had an access staircase down to a trench where I was building a cistern. Once the river wall was breached, water flooded in and my miners evacuated up the stairs. Now I'm left with a staircase that I can't get rid of, since any dwarf that removes it falls into the cistern and can't get out.

I had this problem too. I was building a relatively tall tower, and soon noticed that several of the walls weren't generating a walkable tile on the next floor up. Where there should have been walkable floor, there was 'Open Space'. Also, if I checked the 'Visualize' (V) mode, there were gaping holes in the side of the tower where this bug was occurring.

I just decided to 'b) Remove the offending walls and build them again.' It wasn't a huge inconvenience, but it was fairly frustrating to notice it happening again and again, forcing me to waste time destroying what I'd just built.

I think this was mostly happening on the external west side of the tower. I don't recall seeing any at all on the east side, but maybe it was just a coincidence.

After a bit of testing, it appears that there are "chunks" of tiles for lack of a better way to describe it that refuse to even be open space, instead being blank as if they were hidden, but not. Ah well. I put some spikes at the bottom and had all my dwarves stand on a trap door above it while one of them pulled the lever, thus ending that fortress.

I'd like to reitterate this bug.

Fortifications NEVER create floors on the level above.

Maybe it's not a bug? Fortifications seem like they'd be too hole-y and weak to be walked on safely.

Draco, that doesn't really look like an arrow slot... where would the archer stand?  More likely that's a drainage channel or some such, so that the rain doesn't flood the roof and cause a collapse/major leaking.

I just grabbed a picture where I could see the top-side, instead of only the 2 foot by 2 foot section that is JUST the hole in the wall.

Well, it DOES look like you could stand on the other side and get a pretty good field of view.

Same "no floor above walls" bug here, seems to always happens in the same areas...

btw, the "no floor above built fortification" seems to be a feature, not a bug  :p
