When building a channel indoors, it is probable that the miner will have to go for a little swim. This is not a bug. However, it leads to a set of circumstances that will cause the miner to sit in the same square of the channel and drown to death. The following criteria must be met in order for this to happen:
1) There must be some tiles designated to be mined
2) Your miner must be inside a water filled channel
What happens is the miner attemps to go to the designated mining spot and mine out the designated tiles. Unfortunately he cannot, and as a result you will be bombarded with the following cycling announcement spam:

Since the miner cannot access the tile squares due to his current location, he remains inside the channel. The moment you undesignate all of your mining tiles, he will quite easily find his way out of the channel.
This occurred to me when the designated mining areas were not reachable at all such as when you're planning your next rooms but you don't connect them to your hallways so the mining cannot start.
