Anyway the solution is pretty simple, allow players to designate different parts on any living (or unliving) creature as requiring surgery, that way it could be used to amputate an animals bad leg or dissect a prisoner... It'd be entirely the players choice.
Of course, I can see it now...
Healthcare -> Goblin Prisoner #352318 -> "Treatment" -> Removal [right lung, all] -> Anesthesia: None -> Sutures: None -> Dressing: None

You could still have the automated surgery appointments to avoid massive amounts of micromanagement, but manual control would also be possible.
Which brings me to another idea... transplants! We could have permanently damageable organs which need replacement.

Or make a tame elephant feed a very small fort for a much longer time without any meat going rotten?

I would love to experiment on useless nobles aid my poor overworked rulers when they are injured.  I'd also like to practice on corpses, gobbos and elves.

There was a bit of a problem regarding flashfreezing, a ballistae, and a barrel of dwarven ale. Gonna fix it up.
