Do I have to keep changing manually between training/real weapons or can they have both?

You can use real weapons all the time in the newer versions, even for training.

Well according to the resident sages, if a dwarf is equipped in full metal armor, training accidents are exceptionally rare. When under the inactive schedule, they just perform weapon drills, so no contact is involved.
 In the Inactive, they do gain weapon skills, no matter the weapons material. The training weapons main use is to allow them to be able to practice with weapons cheap to buy/make, until you acquire really good weapons for your military, and armor to outfit your squads in.
 So the answer is really just outfit your full time squads with metal weapons, as long as good armor is provided. If not, might as well keep switching them out, for the active squads. And about outfitting them with a training, and combat weapon, you can, but they will always use the most valuable one(usually metal).

Training accidents used to happen, but not in newer versions.
Dwarves will never be injured when sparring, even if they have on armor at all.
