Can you swap out their gear? Are they doing it themselves while I'm not looking? Or are they basically stuck with whatever they had when I recruited them?

For me it looks like they remain stuck with it , I would wish they would pick up things thought... and be able to use the damned throw skill

In the current version you can use DFusion to manage companion's equipment by temporarily swapping bodies with them. There's no guarantee that this will continue to be possible in the future versions but at some point it will be probably possible to do it without outside tools.
