Outpost liason "leaves unhappy"

My fortress had a tragic accident of stupidity in it's first year and lost it's miner at the bottom of a lot of water just after the caravan had left with the liason. So, I intended to talk to him the next year and ask for more picks (already at my 15-dwarf pop cap, so no immigrants) but when he arrived, right after I hit the continue button I got a message that "a diplomat has left unhappy". My liason was gone. The traders continued on to the depot, but I was unable to ask for replacement picks.

I hope he actually decides to talk next year...

My experience involves the liason going berserk and seriously pissing off one of my dwarves.
This happened after he 'left unhappy' then never left.

This means - Last time I had it happen - That there was no noble for the liason to meet with, meaning that the miner that died might have been the outpost manager/expedition leader/whateveritishemeetswith.  Try reassigning someone to that position.

Draft your outpost liason into military or station him at trade depot, that way the other liason can get to him.

This may have been it -- if I'm recalling correctly my leader decided to walk in front of the ballista while my operator was practicing. I wasn't able to appoint a replacement either. Each time I tried, as soon as I left the nobles screen and returned the place would be empty again. They did just elect a new one though.
