I did this for my hospital, seemed to do nothing for infection or cleaning patients.

The point isn't to do anything for infection or cleaning patients, but rather to help mitigate and contain syndromes. When a dwarf goes through the bath, all contaminants on them are removed and transferred to the water, including things like Forgotten Beast extract which could otherwise be left in random, deadly puddles scattered about your fort as the dwarves who have them on clean themselves at a water source or die from the syndrome. If your dwarves are going to the hospital, it's probably already too late to save them, so that's why you put your dwarf bath at the cavern entrance(s) or on every main thoroughfare.
