Did this happen right after it gives you the intro paragraph (after embark)?  I've seen that reported a few times, but I have no leads on it.

It just did that for me... I can tell you about the start, in case it is somehow related to that.

I have no trade with all civs but goblins

Tundra
Temperature: Freezing
Trees: None
Other Vegatation: None
Surroundings: Untamed Wilds

2 x Proficient Miner
2 x Proficient Mason
1 x Proficient Mechanic
1 x Proficient Grower
1 x Proficient Axedwarf

2 picks
1 axe
6 plump helmet spawn
5 pig tail seeds
15 turtle meat
15 plump helmet

2 dogs
2 kittens

One other thing to note is that the location is right on the edge of the map.

I couldn't get it to repeat with the same setup, but maybe this will help anyway.

Yupp, same thing here:

Latest version (0.22.121.23a)

This has happened twice:
(Setup is clean install of new version into empty folder)
- I create world in windowed mode.
- When finished (F11)
- Start new game
- Select site/skills/equipment
- (e)mbark -> intro paragraph
CTD

Upon restart of the game after the CTD the created world works fine for same initial setup.

The F11 doesn't seem to be enough to trigger it.  I've been playing with this for an hour and I can't get it to happen at all.  It doesn't help that it doesn't reproduce with the same parameters.  After the intro paragraph shows up, everything is in place, so it's only when all of the game routines starts running that it crashes.  That doesn't help to narrow things down.  It's either some random chance associated to how the starting build is placed (or those units are generated), or some random chance associated to how it lays out the map, I think.  It's too much to check manually though, so I just have to wait for it to happen to me so I can break out in the proper location, unless there are more leads.

If it helps, I've seen this same CTD not only when starting a new game and/or loading save games in Dwarf Mode, but also when entering towns, ruins, etc in Adventurer mode. Maybe it's a generic "map loading" problem?

Okay, I've got it to happen, and it was enough to figure it out.  It'll happen sometimes, and you definitely have to follow the create world + play immediately routine.  Occasionally some world gen info was persisting that confused the equipment pickup routine for the initial miner...  I haven't fixed it yet but it should be pretty easy.

Any other CTD is different from this one.  There are many possibilities for catastrophic failure, many of which I've fixed now.  Hopefully I can get this up tonight once I figure out exactly how to fix it.

Maybe related, I downloaded the first pregen world, played an adventurer in it, then played a fortress, and crashed to desktop after the embark paragraph.

Hopefully this is fixed as well.  The latent information that was causing the crash would last through an intervening adventure mode game without causing trouble there (or possibly causing some trouble...).

With the last version, haven't tried embarking on this one, I usually crashed every second time I try it. So, when I start up a Fortress Mode I just press embark instantly, crash, then I can redo it properly.
