Military gear randomly becoming forbidden?

I play pretty fast-and-loose with my military uniforms. If I'm still in the early days, I let my soldiers equip pretty much whatever they want, but even when I get a metal industry up and running, I don't bother making my own weapons. I let them choose their own.

Lately I've noticed that my military dwarves are dropping their flasks a lot, and when that happens, they show up as forbidden. Also, I've sometimes checked out a dwarf's inventory and noticed that its weapon — which it's still carrying — has become forbidden.

This doesn't seem to hurt anything, but I do need to periodically look around my fort for bits of whatever that've been dropped and are forbidden.

Is this a bug? Or is this me doing something wrong?

I am pretty aggressive with my mass-designation commands, both through the d-b menu and through the stocks screen. But I'm almost certain I'm not doing this manually. It seems to be happening automatically.

Any ideas?

The weapons are being forbidden when they get stuck in an opponent. Dunno about the flasks, though.

  Ah, that explains why. My military won't drop the stuff though unless they go into the hospital. I really hate seeing that super high quality sword with a semi-fitting name sitting on a table in the hospital. "Mulash Meng: The Confederacy of Lashing"
