Well, just do whatever you feel is best. Also, it's actually a good thing that your girlfriend dumped you over this, now you know that she was  shallow and did not have to spend more time on a relationship that was not going to work out.

Well... damn.

You could go the cryptic route.  Be like "Hey, I got diagnosed with hemoheartophillia (or whatever the technical name of your condition is) and so I can't do anything that gets my hear rate up"

They'll just glaze over name of the condition, and probably won't even bother to look it up later (unless you e-mail them, in which case they'll just copy/paste into google).  Then, if they ever ask you why you didn't tell them you have a hole in your heart, you can honestly say "but I did tell you I had hemoheartophillia"

A friend of mine got a pretty nasty back problem which, while not lethal, may leave him paraplegic.
He began by a few understatement ( something like "I've go a little health problem") and developed as it get worst/ got question about it. It worked well.
