What have been the significant changes in the game since the update that had the caverns? The devlogs are TL;DR.

Adventure mode is awesome now.

And don't disrespect the dead, FUN lies that way...

Dwarven blood types are not A, B, AB, O but Ale, Wine, Beer, Rum, Whisky and so forth.
It's not an embark so much as seven dwarves having a simultaneous strange mood and going off to build an artifact fortress that menaces with spikes of awesome and hanging rings of death.

Not a lot has changed in Fort Mode. Just some new surprises if you don't bury your dead properly.
Adventure mode has received a good bit of love, but there's still a good bit more needed there before it can be considered a truly great game.

If you want more than that, look here:

Everything that's green is in. Everything that's purple is in in some form. It's not that much, really.

The "Dwarf Fortress version X has been released" threads in the DF Announcements board list all notable changes (and the most recent one is always linked at the top of any forum page).

And if it wasn't clear, most of the bugs have been fixed.  There's an annoying one related to dwarves not resting during surgery and it can be difficult to get dwarves to wear two gauntlets, but as far as I know all the other equipment bugs are fixed, there are no more unkillable megabeasts, etc etc

There have been several hundred bug fixes, with each release averaging to 18.4 bugs being fixed.

Crutches still don't work in fortress mode :-(

The entire content consists of senseless murder, a pile of faceless naked women and zero regard for human life in general, all in the service of the protagonist's base impulses. It is clearly a cry for help from a neglected, self absorbed and disempowered juvenile badly in need of affectionate guidance. What a sad, sad display.

After.19 release, it was stated that the .20 would be a bug fix release.  THEN ONTO ARMIES!

Not exactly. Toady said he's going go get a lot of bugfixing done after caravans, that's all. Caravans could be broken into more than one release, and bugfixing almost certainly will be. There will probably at least be a little quick-fix update after the caravan release(s), like there has been after all the last few major releases.

He he he.  Yeah, it almost looks done...  alas...  those who are in your teens, hold on until your twenties...  those in your twenties, your thirties...  others, cling to life as you are able...
It should be pretty fun though.
