Dwarves chasm external refuse

When chasming particular refuse is on, dwarves gather outdoor refuse to chasm even if collect external refuse is off.

Additionally, if the particular part of the chasm they want to go to is blocked, they just loop in place until they have to eat or sleep (instead of finding another chasm to throw the kobold skull into).

The first part is Bug 95, and I'll make a report for the other one.  I think I've seen it in a save somebody sent before.
