I don't want to discourage you from filing bug reports, but could you search the existing bug reports before filing new ones, to see if it's already been reported?

Anyways: If all your soap isn't in the hospital, animals in particular will steal soap out of the stockpiles, carry it away, and then drop it and spam this error message. I haven't seen dwarves do it yet myself. Your dwarves will then carry it back to the stockpile, where the process will repeat. The only solutions are to either forbid the soap or increase the amount of soap allocated to the hospital and give it enough containers to hold it all.

I went ahead and relocated all my soap to a new stockpile in the hospital, and I haven't gotten that annoying message again.  I assume that means that once I get a dwarf that's injured, the soap will be used on them.  All the dwarves I had from the last wave of carnage have died already, and are beyond the healing powers of scrubbing bubbles.

I made about 20 buckets of lye and had a stockpile for just lye filled with buckets, and no barrels.  I ordered up soap through the manager and ignored about a year of error announcements.  Then suddenly, miraculously, my dwarves produced two bars of soap.  I say miraculously because there is no indication if they had actually managed to fill a barrel with lye.  Whatever worked for those two bars wasn't happening continuously because only two of my order of five were made.  Anyway, now I'm trying to get my dwarves to put the soap in my hospital so they will stop playing pass the soap.  I think the problem is that because so many need to use it or want to use it, the soap keeps getting tasked for one of the useless jobs, and so its never available for a dwarf to get the job to move it to the hospital. 

The hospital says   Soap: 0/5700 (whatever this means)

I've also got tons of available coffers in the hospital, which the dwarves have been filling with useful supplies.  I've even got them to put the soap in a stockpile inside the hospital, but that doesn't seem to help.  Any ideas on how to get them to store the soap?

On the plus side, my team of doctors fully healed a guy with three compound fractures, so that was very encouraging.  His buddy is in infinite surgery though.

On my fortress, I was only able to get the dwarves to stock soap in the hospital by building a lot of coffers in the hospital zone.  It seems that hospital supplies take up a lot of room, and they won't store soap until everything else is stored.  Try building ten or so more coffers in the hospital and see if that does it.

I was also getting the "Cancelled clear self: area inaccessible" spam until I created a new water zone atop a brook that they all could reach.  That might have been unrelated however, as this was about the same time I managed to get them to store the soap properly.

I've had a similar problem, except it was with clothes instead of soap. For some reason, over a dozen dwarves stood around the Finished Goods stockpile taking clothes out of bins, dropping them, putting them back in, taking them out, putting them back in...

I started forbidding everything 'cuz it was ticking me off.

update, it seems that if you build plenty of extra coffers and wait, eventually soap will get placed, resolving the issue.  So far they've stored one of the bars, so hopefully they'll do the other one soon.

What is plenty of extra coffers?

I got 15 coffers in my hospital and if I put more - the chances are that they get filled with cloth/thread before dwarves figure out there's soap to carry in there too.

I currently have 180 000/75 000 thread and 410 000/70 000 cloth in my hospital.. Cant wait for this bug to be squashed that dwarves would actually honor the limits..

I restrict all of my soap to a single stockpile only accessible by crossing a 3/7 pool of water.  By the time my dwarves reach the soap, the water has already rinsed them clean!

today i had this one particular child that would not get clean, nor could figure out how to find the soap.  After a year of being spammed with the messages, i assigned him to a burrow placed tragically under a drawbridge.
