Anyone else have issues getting their dwarves to eat / drink? My military especially is going to the point of death before eating. I disbanded the squad and they went to eat, but even after that they are still starving most of the time. Any ideas what I could be doing wrong?

Okay, I found the cause, just not the why. They wouldn't eat while I had merchants in the fort. I'm guessing a 1 tile space between the depot and food stockpile isn't enough to walk through.

Tell your soldiers to carry food in the supplies screen.

I had trouble with military guys taking prepared food and leaving it standing around without eating it or carrying it.
