Water infection

In medieval times, they didn't drink 100% water cause it was spreading infections. So, if your dwarwes drink 100% water, they could get infections. Or maybe they could mix 50% booze and water too. Even children had small amount of wine in their water for drinking  :)

So that's why dwarfs drink only alcohol.

I imagine it kills off any bacteria very well.

This seems as though it would be particularly relevant on the 'evil' locations, where (I assume) the water is supposed to be foul and unfit for drinking. We'll probably get something like this when diseases and poisons and the like are added in.

i would not like to see plagues and sickness installed anytime soon
its already hard enough to get 30+ dwarves to do what you want but to have 15 of them sick with the flu in bed all the time would be a real hassle
not to mention being wiped out by human caravans carrying the plague

Even Dwarven babies and children require alcohol to get through the working day!

I don't think it's a mere addiction, I think the dwarven biology requires alcohol for some metabolic process. See how they slow down without it - their metabolisms apparently slow down.
