[I'm using 40d with the Mayday mod, as downloaded from TinyPirate's tutorial pages]

I had a huge goblin battle in the Spring of 208, in which my Broker/Hoardmaster broke a leg and was maimed in the foot. She immediately went to sleep in the barracks and went into a coma.

Come summer, the Human Guild Representative arrived with the caravan. I did all my trading, but then I noticed that no meeting had occurred. I checked on my Broker and, of course, there she was, still asleep in the barracks, with the Guild Representative patiently waiting for a meeting at her bedside. So I quickly replaced her with another dwarf as Broker (including handing over her office to the replacement), hoping that would fix the problem.

It did not. No meeting occurred. It is now Spring of 209, and the Human Guild Representative is still standing around in my barracks, waiting for the former Broker to wake up. As it turns out, her foot has healed completely. Now she just has a broken leg, so there is a good possibility she will wake up within a few months. I will post with updates as things progress...

Sounds like this bug:

# 000589 □ [dwarf mode][diplomacy]   (Report) unconscious dwarves conduct meetings, and the meetings don't advance while they are doing this

After reading the report for Bug 000589, it seems it's not the same issue (however, there is no mention of consciousness in the report). In my situation, I wouldn't describe it as a meeting occurring while the dwarf is unconscious. Rather, it's more like the Guild Representative is unable to deselect the dwarf she was programmed to meet with when she entered the map, and she's waiting for the meeting to happen once the dwarf is conscious again. When I select the sleeping broker with "v", it says "rest" as her current task, rather than "conduct meeting."

My guess is that the meeting will begin once the resting dwarf is conscious again. Although I'm not sure. Maybe she needs to be made the broker again for that to happen?

Also, one thing I'm curious about is what will happen when summer rolls around, and the HGR is supposed to visit again. Will nothing change, she'll just keeping waiting? Will another HGR show up? Will her task programming change, and she'll want to visit the current broker? These are things I intend to keep track of and report on.

In the current version, there's only one guild rep, and if they die, you'll never see another.  So fear not, your guild rep will hang out safely in your fortress forever.

Your bug has been reported before -- that changing the noble doesn't redirect the meeting -- but somehow I'm also not finding it in the list.

UPDATE

Just to confirm what was predicted accurately, when my former broker finally woke up in Winter 209, the HGR followed her around and conducted the meeting. I did not need to reassign the former broker to the broker position.
