Is it possible that there is a limit of only one of each of the possible features per 16x16?  Has anyone ever seen, for instance, two magma pipes in the same embark 16x16?

You get:
One pipe and one pool per region tile with 80 or more volcanism anywhere on it
One HFS zone per mountain region tile
(I think) one chasm and one cave river per mountain region tile
I suspect that you get a bottomless pit and a pool as well, but since I don't see anything other than the magma features in the finder I'm not sure.

Normal rivers aren't limited, but I think that the rest are at most one per region tile.

Rysith is right. You only get one of every feature EXCEPT above-ground rivers, you can have multiple of those. So...

1 Magma Pipe
1 Magma Pool
1 Chasm
1 Bottomless Pit
1 Underground River
1 Underground Pool
1 Hidden Fun Stuff
1 Enemy Fortress

Above-ground rivers is limited to what it is you want to take the time to look for.
