I've always built an exit from my wells, because of long-standing rampant warnings about well-divers.  Oh, the dwarfanity.

But I don't think I've ever seen a dwarf go down one of my wells, or even show up inexplicably wet.

Is this precaution just an unnecessary legacy?  Or is it only if you have wells in barracks, or have combat near them?  Anyone suffer from well-divers, and if so what were the conditions that seemed to spark this lemming-like frenzy?

I believe it's only if combat occurs near them, these days. I know that's the case for the 15 z-level drops that are often near my stairs.

My well shafts went near my barracks.  I would occasionally (twice a year or so) have a sparrer fall down the shaft.  Landing in water didn't seem to hurt them, so I just put ramps nearby for them to get out, and all was well.

Besides that, the only dwarves I've ever had dive down a well shaft have been melancholy.

Go ahead and make wells with no exits, I've never had a problem.
You can go ahead and make all your stairwells use 20 levels of up/down stairs, too.

Yeah, the only problems I've had with absurdly tall up/down stairs is when a cave-in occurs and miners get tossed down them - then that drop is painful.  But hatches can prevent that, so that's covered as well.
