One of my favorite mods for Morrowind was Necessities of Morrowind, which adds those survival aspects.

Travelling for more than a day required you to lay out your bedroll, light a fire, cook dinner, and get a good night's sleep, else suffer stat decreases, and eventually death.

Meals were as simple or as extravagant as you wanted, from just roasting an ash yam, to adding an ear of corn, and even consulting a cook book to collect ingredients and utensils to even make breaded chicken starting with a raw leg and whickwheat.

Makes you worry about time of day, your water or booze supply, substantial food, and overall adds a lot to the game.
Major mods like Tamriel Rebuilt usually add support for NOM.

You could also get really drunk and pass out :)

You know, I'm pretty sure it's easy to add something like that to EVERY RPG through a few variables. I'm also pretty sure you could scale that, with something resembling a difficulty slider from (turn it off) to (full effect).

Few hours of development work, catch a broader target audience.

Sometimes I think you're not allowed to develop games with a three digit IQ...
