some thoughts:
'q' over it to find out what's going on. i get the impression you have done so though if you are not receiving the dry message, and are instead receiving the active message.

have you designated the well area (well plus surrounding tiles) as a watersource via 'i'? (you could further encourage the dwarves to take water from the well if you toggle on the 'o'rders menu 'zone-drinking only').

have you tried deconstructing the well, and rebuilding, checking for a channel above the water source, making sure you have right stuffs to build the well, and that you have a spare bucket or two?

that's my thoughts - i'd gamble on the second option being the one most likely to help....

Do you have spare buckets?   In addition to a water source a dwarf needs a bucket to carry the water to the injured dwarf.

Buckets are the usual problem here. Tell your carpenters/metalsmiths to make a whole bunch more. They'll only use buckets that are not being used in a job, and jobs include the bucket scheduled to be hauled back to storage

i'm pretty sure it's to do with the act'i'vity zone, or that maybe there is no water below the well. but it is unlikely to be the latter if the well is showing up as 'active' under a 'q'uery. so i'm gonna go with the 'i' theory, still...i've thought about it some more, and i'll test it when i'm home in an hour, if i can.

I like to keep about 10 buckets around when I have no water hauling tasks, and keep them stored by the well.
